Mana is stored in the testes for men and ovaries for women. Enuchs, sterile men, barren women and transwomen don't have any means of storing enough Mana for even the most basic of divination. This usually results in the lack of an "internal voice" since Mana cannot be held and thus provide insight into the otherworld. (Though there are reports of otherwise healthy individuals not experiencing contact with spirits.) This means that "transitioning" from male to female, cutting you dick off, translates into losing your entire Mana pool. Whereas a vasectomy or tube ligation just means you are cut off from it but access can be restored or Mana used in other ways like making charms (see testicle jewelery in Chapter 6M: Jewry).
